The Tarleton Alumni Association honors individuals each year as Distinguished Honorees in several categories: Alumnus, Outstanding Young Alumnus, Friend, Faculty and Staf. The honorees will be honored at the Distinguished Alumni Gala in January/February 2012. The TAA recognizes that there are numerous individuals that are worthy of these designations and it is a difcult task to make the annual selections. No selection committee can be aware of the many and varied contributions made to Tarleton by such a large group of potential honorees. We need your help in selecting our 2012 honorees. Please provide your recommendations to the committee for our deliberations. The recommendations are needed by June 15, 2011. We would prefer that you limit your recommendations to two or three individuals per category. Please complete the following nomination form for each nominee and provide supporting documentation as required for each category. Feel free to make copies of the form for each nominee. Awarded to an alumnus who has received signifcant recognition through career, community service, or leadership position(s). Please provide detailed information about the nominee including: • Involvement with Tarleton after graduation • Current and past employment, listing honors received

• Community involvements/leadership positions held, state or national involvement • List of family members who attended Tarleton and any presently attending Tarleton

Awarded to an alumnus, under the age of 40, who has received signifcant recognition through career, community service, or leadership position(s). Please provide detailed information about the nominee including: • Involvement with Tarleton after graduation • Current and past employment, listing honors received

• Community involvements/leadership positions held, state or national involvement • List of family members who attended Tarleton and any presently attending Tarleton Note: Must be under the age of 40 and a Tarleton Graduate

Awarded to a former Tarleton faculty member who had a distinguished career at Tarleton and/or received special recognition since leaving Tarleton. Please provide detailed information about the nominee including: • List of honors that helped Tarleton grow as an institution • List of actions this person performed that beneftted Tarleton

• List of family members who attended Tarleton and any presently attending Tarleton Note: Nominee can’t be a current faculty member at Tarleton

Awarded to a former Tarleton staf member who had a distinguished career at Tarleton and/or received special recognition since leaving Tarleton. Please provide detailed information about the nominee including: • List of honors that helped Tarleton grow as an institution • List of actions this person performed that beneftted Tarleton

• List of family members who attended Tarleton and any presently attending Tarleton Note: Nominee can’t be a current staf member at Tarleton

Awarded to an individual or organization who has brought recognition to Tarleton or who has supported provided signifcant support to Tarleton or the Tarleton Alumni Association. Please provide detailed information about the nominee including:

• Description of involvement with Tarleton and/or the Tarleton Alumni Association • History of contributions/support made to Tarleton and/or the Tarleton Alumni Association • Current contributions/support being made to Tarleton and/or the Tarleton Alumni Association • List of family members who attended Tarleton and any presently attending Tarleton
